Who we are

Coordinated entry was a process developed to ensure that all people experiencing a housing crisis have fair and equal access and are quickly identified, assessed for, referred, and connected to housing and assistance based on their strengths and needs.
Coordinated entry guides anyone experiencing homelessness to the correct support and housing program to end their homelessness permanently. Our region consists of Delaware, Dubuque, Jackson, and Clinton counties

Whether it’s a place to live or people to provide support, Coordinated Entry provides fair and equal access to all those in need of housing and assistance based on their strengths and needs.

Terms

Permanent Supportive Housing: is permanent housing with indefinite leasing or rental assistance paired with supportive services to assist homeless persons with a disability or families with an adult or child member with a disability achieve housing stability. (HUD)
Chronically Homeless is a literally homeless individual with a disabling condition who has been continuously homeless for a year or more, OR who has had at least four episodes of homelessness in the past three years. (HUD)
Literally Homeless is any individual who is residing in a shelter, Safe Haven or place not meant for human habitation. (HUD)
Rapid Rehousing is a program that rapidly connects families and individuals experiencing homelessness to permanent housing through a tailored package of assistance that may include the use of time-limited financial assistance and targeted supportive services. (HUD)
Street Outreach is an essential service related to reaching out unsheltered homeless individuals and families, connecting them with emergency shelter, housing, or critical services, and providing them with urgent, non-facility-based care. (HUD)
Unsheltered Homeless is an individual or family who resides in a place not meant for human habitation. (HUD)
